SITE MONITORING REPORT ERRORS

We are expanding the scope of the work for the firm contracted to review RBCA Tier 1 and 2 Reports to include the review of reclassification Site Monitoring Reports (SMRs). Initially the contractors reviewed 45 SMRs submitted for 27 LUST sites. The following is a summary of common errors contained in the reports. Please check your work to avoid these errors. Tracking the submittal of reclassification SMRs will be an item of emphasis. Please submit the reports as scheduled.

  1. Vapor Sampling Results
  2. Soil source submerged when the vapor sample was collected.
    Vapor sample was collected at the wrong depth.
    Incomplete information (e.g., Static water levels not provided).
    Confirmation vapor sample was not collected.

  3. Tier 2 Report deficiencies were not addressed in the SMR or the method of correcting the deficiencies was inadequate.


  4. Remediation done at the site or the institutional control/restrictive covenant proposal lacks documentation.

  5. Plastic water line replacement not documented.
    Inadequate documentation of overexcavation.
    Documentation of water well ordinances not provided.

  6. Monitoring plan or exit monitoring criteria not met.

  7. Monitoring wells or vapor points listed in the monitoring plan were not sampled.
    Confirmation sampling not conducted.

  8. Site conditions (e.g., Free product was discovered, groundwater maximum increased >20%) warranted a site re-evaluation. Re-evaluation was not conducted.



  9. Soil replacement sampling was inadequate.

  10. Documentation of a submerged soil plume was inadequate.
     
  11. High risk drinking water wells or plastic water lines not sampled.
