No authorizations will be issued or reissued under GP #5 until GP#5 is renewed. To obtain a wastewater discharge permit for your quarry operation, please apply for an individual NPDES permit using Industrial Wastewater Application Forms 1, 2, 5, Supplement for TDS and Chloride Data, and Application Fees Invoice (542-1253). All these forms can be found on this webpage. Please apply for the permit at least 180 days before your planned discharge.
